4.2 Posology and method of administration Posology As the cardiovascular (CV) risks of celecoxib may increase with dose and duration of exposure, the shortest duration possible and the lowest effective daily dose should be used. The patient's need for symptomatic relief and response to therapy should be re-evaluated periodically, especially in patients with osteoarthritis (see sections 4.3, 4.4, 4.8 and 5.1). Osteoarthritis For relief of the signs and symptoms of Osteoarthritis the recommended oral dose is 200 mg per day administered as a single dose or as 100 mg twice daily. Rheumatoid arthritis For the relief of signs and symptoms of Rheumatoid arthritis the recommended oral dose is 100 to 200 mg twice daily Special populations Elderly Dose adjustment in the elderly is not generally necessary. However, for patients of less than 50 kg in body weight, initiate therapy at the lowest recommended dose. Pediatric population Celecoxib is indicated for use in adults above the age of 18. CYP2C9 poor metabolizers Patients who are known or suspected to be CYP2C9 poor metabolizes based on genotyping or previous history/experience with other CYP2C9 substrates (such as warfarin, phenytoin) should be administered celecoxib with caution. Consider reducing the dose to half the lowest recommended dose (i.e., CYP2C9*3/*3) (see section 5.2). Hepatic impairment The daily recommended dose of Celcox capsules in patients with moderate hepatic impairment (Child-Pugh Class B), should be reduced by 50%. The use of Celcox in patients with severe hepatic impairment is contraindicated. Renal impairment Celcox is not recommended in patients with severe renal insufficiency. Method of administration Oral use Celcox may be taken with or without food. The capsule should be ingested intact with a glass of water. No information is available regarding the dispersion of the capsule content.
